Support for a Lifetime

When the support team, which includes parents, staff, other family members or outside professionals, agrees that the participant has achieved all the benchmarks for transition to independent living, a transition plan is adopted. The participant then moves out into the Greater New Haven Community and receives support from the Asperger's Community Support Program for a minimum of one year. This support will serve to ensure that the strides made during the residential program are mainatained during the critical months following the transition. However, support services are recommended for a longer period than the required one year and are delivered in the amount necessary for the success of the individual.
