Dan Forman M.S., CCC-SLP

Dan Forman, MA CCC-SLP, has been the speech-language pathologist for the REACH program since 2012.

At Chapel Haven, Dan focuses on social communication and helping individuals better connect with peers, family, staff and employers as they take on increased independence and responsibility.  Through close collaboration with the SAIL, CLP and SLP programs, he is able to support individuals through the various stages of their Chapel Haven lives.

Dan received his Master’s in Speech Language Pathology from Vanderbilt University and his BA in English from the University of Pennsylvania.  While at Vanderbilt, he was a recipient of one of twenty grants from the U.S. government dedicated to creating leaders in the language and literacy field. Prior to becoming a speech-language pathologist, Dan earned a Masters of Fine Arts in Screenwriting from the University of Southern California and had a long career as a screenwriter.  Dan has written and rewritten over a dozen projects for studios and producers including Warner Brothers, Disney, NBC Television and Imagine Entertainment.
