Community Outreach Services (COS)
Chapel Haven Schleifer Center’s Community Outreach Services offers individualized in-home and community-based support for adults who have completed the ASAT or CATCH Programs.
Our dedicated staff partner with each individual to promote independence and to achieve their own unique goals, offering case management and life skills support ranging from 1 hour per month to up to 6 hours per week.
While most COS community members live off campus, they enjoy full access to Chapel Haven’s vibrant community, including recreational activities, wellness & fitness options, and numerous special events. In addition, adults may choose from a wide range of a la carte services, such as:
- Employment & volunteer services through CareerAbility
- Supports for College
- Executive Functioning Coaching
- Social Communication Coaching
- Continuing Adult Education classes
- Recreational Programming
- Clinical Services
Combined, these supports create a comprehensive, lifelong pathway tailored to each individual’s needs. For peace of mind, the program also offers access to 24-hour emergency assistance during overnight hours.
